YOU/YOUR AUTHORIZED/LEGAL
REPRESENTATIVE ARE RESPONSIBLE FOR:
HEALTH MAINTENANCE OF SELF,
FAMILY AND COMMUNITY, through participation in the varying
health promotion/illness prevention and curative/rehabilitative
programmes offered in the public or private health sector.
SUPPLYING INFORMATION
that as far as possible, is complete and true about the present
complaint; previous illness and treatment; family history and social/economic
matters that may be related to his/her health.
COMPLIANCE
WITH INSTRUCTIONS:
the treatment recommended by the
care giver.
keeping your appointments.
the rules and regulations of the
health care institution.
care of the health care institution’s
property
the rules relating to the client's
behaviour.
keeping all documents in a safe place
and presenting them on request.
YOUR ACTIONS RELATED TO THE
REFUSAL OF TREATMENT.

are OBLIGATED/ LIABLE:
TO PAY ANY COST INCURRED IN MEETING
YOUR HEALTH CARE NEEDS. There are mechanisms in place to ensure
that clients who are unable to pay receive health care in the public
sector.
FOR ANY EQUIPMENT, LAUNDRY, PHARMACEUTICALS,
OR OTHER SUPPLIES
taken possession of without authorization
damaged by you/your authorized/legal
representative.

EXPECTATIONS
You/your authorized/legal representatives
are EXPECTED TO:
DEMONSTRATE COURTESY AND
RESPECT FOR ALL HEALTH CARE DELIVERY PERSONNEL
DEMONSTRATE MATURE, RESPONSIBLE BEHAVIOUR
by giving advanced notice to cancel an appointment when you are
unable to keep it.
ENCOURAGE HEALTHY PEOPLE
TO DONATE BLOOD OR ORGANS SUCH AS KIDNEYS IF THEY DIE IN AN ACCIDENT
WHERE THIS DOES NOT CONFLICT WITH PERSONAL/RELIGIOUS BELIEFS.
