Jamaicans Trained in Jamaica - Each applicant must submit a completed application form (ensure that the form is properly completed).
- One passport size photograph.
- Two letters of reference (one MUST be from someone in a supervisory capacity and a member of the profession in which the applicant is applying. The other must be a character reference from someone who has known the applicant for a number of years. These letters must be addressed to the Registrar, properly dated and signed). References must be dated within a six-month period.
- Copy of Diploma/Degree (certified by a notary public).
- A fee of Two Thousand Dollars ($2,000.00).
- Letter stating completion of Internship (this is required from Physiotherapists, Medical Technologists & Medical Dietitians. Internship is not required for a Radiographer; however, a letter must be received from the Director of the School of Medical Radiation and Technology, stating that the applicant has completed the course of study).
- Medical Technologists must produce proof of subjects gained to enter the Medical Technology programme. They must be able to show five passes in GCE (A, B, C, D); CXC (1, 2 prior to 1998 and 3 after 1998. These must be certified copies).

- Two letters of reference (one MUST be from someone in a supervisory capacity and a member of the profession in which the applicant is applying. The other must be a character reference from someone who has known the applicant for a number of years. These letters must be addressed to the Registrar, properly dated and signed). References must be dated within a six-month period.
- Original Copy of Diploma/Degree (certified by a notary public). This will be returned.
- A fee of Two Thousand Dollars ($2,000.00).
Foreign Nationals - Each applicant must submit a completed application form (ensure that the form is properly completed).
- One passport size photograph (certified by a notary public).
- A fee of Sixty US Dollars (or the Jamaican equivalent).
- Original of Diploma/Degree (certified by a notary public). This will be returned.
- Two letters of reference (one MUST be from someone in a supervisory capacity and a member of the profession in which the applicant is applying. The other must be a character reference from someone who has known the applicant for a number of years. These letters must be addressed to the Registrar, properly dated and signed). References must be dated within a six-month period.
- Copy of the applicant’s current license to practice (certified by a notary public).
